Job Title : Staff Social Worker

Job Description :

We are seeking a compassionate and dedicated Staff Social Worker to join our team. In this role, you will provide essential social work services to assist patients facing social, emotional, and related challenges associated with their disabilities. You will work within established routines while having the flexibility to determine the scope of work necessary to help patients adapt to their environments.

Key Responsibilities :

• Provide social work services to patients with disabilities, addressing their social and emotional needs.
• Collaborate with supervising social workers to ensure effective service delivery.
• Meet with community professionals to obtain assistance and guidance for patients.
• Develop and implement strategies to help patients adapt to new or existing environments.
• Maintain accurate records and documentation of patient interactions and progress.
• Participate in team meetings and contribute to the development of care plans.

Requirements :

MINIMUM QUALIFICATIONS : Possession of a professional license and current registration issued by the NYS Education Department as a Licensed Clinical Social Worker (LCSW); OR Licensed Master Social Worker (LMSW).

SPECIAL REQUIREMENT : Incumbents will be required to possess a valid New York State Motor Vehicle operator’s license or otherwise demonstrate the ability to meet the travel needs of the job.
