Job Duties :

• Renders services in strict accordance with the written Plan of Care. Reports to the Case Manager when unable to do so.
• Performs simple range of motion exercises and / or assistance in ambulation as an extension of therapy services under the direction and supervision of the therapist.
• Applies safety principles and proper body mechanics in the performance of specific techniques of personal and supportive care.
• Performs personal care activities, including but not limited to bathing, shampooing, skin care / nail care, oral hygiene, shaving, and dressing, assisting with toileting / elimination., transferring and ambulation. Performs light homemaking and other environmental services that facilitate receiving sanitary and safe care at home such as meal preparation / feeding, laundry, and light housekeeping and cleaning up the work area.
• Protects patient’s right to privacy and maintains their dignity while providing personal care.
• Measures and reports vital signs.
• Documents observations and care provided. Submits required paperwork per agency guidelines.
• Perform additional duties and responsibilities as deemed necessary.
