Shift : MondayFriday, daytime hours, with possible on call and limited weekend rotation

Job Description :

Sunbelt Staffing is hiring a Home Health RN to serve patients in the Ronceverte, WV area. The nurse will manage in-home visits, complete OASIS documentation, and provide high-quality clinical care with a focus on patient outcomes and independence. This role is ideal for RNs who enjoy autonomy and community-based practice.

Key Responsibilities :

Perform start-of-care assessments and follow-up home visits

Document all patient encounters via OASIS in Homecare Homebase (HCHB)

Deliver skilled nursing care such as wound care, injections, and chronic condition support

Educate patients and caregivers on home health plans, safety, and recovery goals

Collaborate with physicians, case managers, and therapy staff

Qualifications :

Active RN license in West Virginia

At least 2 years of RN experience, with 1 year in home health

Required : OASIS and Homecare Homebase (HCHB) experience

Current BLS certification

Reliable transportation, valid drivers license, and proof of insurance

Able to pass drug screen, background check, and meet Joint Commission requirements
