About The Position

ID : 72854545 Shift : M-F, 8-5 / 730-430, no holidays no weekends. Maximum of 2 Days Call off (Unapproved RTO) during contr Description : Primary Care Clinic at Tsehootsooi Medical Center.Title : Care Coordinator (case manager) nurse positions within our Primary Care : Three (3) year Case Management Experience.Shift : Weekdays Monday through Fridays from 8a to 5p, with One (1) hour lunch, no weekends or holidays. Collaborate with support nursing staff and with providers for patient care.Housing : Information can be found under the 'Facility Group Document section. Orientation : on Mondays only - If your traveler misses the compliance deadline, they will be pushed to the following scheduled Orientation. Additional are occasionally expected to float to other departments in which they are qualified to work. This could include taking a patient load if experienced / competent in that setting or could be task-oriented such as answering call lights, phones, etc
