QUALIFICATIONS :

Master of Social Work Degree and valid Illinois Professional Educator License (PEL) with Social Worker endorsement.

PERFORMANCE RESPONSIBILITIES :

• Demonstrate accurate, up-to-date knowledge of content and Illinois State Standards
• Participate in building level Learning Center team meetings to assist with creating plans for individual students
• Maintain accurate records and prepare reports for legal action
• Provide appropriate IEP related services based on assessed social / emotional needs. Provide professional social work assessments and evaluations as outlined in IDEA (Individuals with Disabilities Education Act)
• Identify, coordinate and make referrals to community resources for students, families and school programs
• Provide individual and group counseling with students when appropriate to do so in an educational setting and as time permits
• Maintain social work records; coordinate and document transfer of due process and social work records
