• Successful Primary Care group hiring
• Location : LaPorte, Texas
• Outpatient Quality Care / Managed Care
• In Office -M-F : 8a-5p
• No weekends
• Low patient panel
• Great Work / Life balance
• Comfortable with use of Electronic Medical Records (EcW)
• Detect and manage chronic diseases
• Develop patient strategy to reduce hospitalization and ER visits
• Collaborate with Case Managers and Specialty Providers
• Educate patients on the value of compliance
• Compensation & Benefits :
• Competitive base salary of $230,000"$290,000 (depending on experience)
