Job Description

A supportive housing program in New York City is seeking a Senior Case Manager to join their team. In this role, the Senior Case Manager will provide case management, housing support to participants and monthly reporting requirements that will ensure compliance with the parameters of the program.

About the Opportunity :

  • Start Date : ASAP
  • Assignment Length : Temp with potential to perm
  • Schedule : Monday to Friday
  • Hours : 9am to 5pm

Responsibilities :

  • Serve as the primary liaison between participants, Superintendents, and Building Managers to ensure timely completion of all apartment and building repairs
  • Maintain a detailed building / apartment log, including dates and condition reports for units and common areas
  • Identify and report non-compliance issues related to apartments or buildings to the Program Director
  • Deliver case management services and provide on-site crisis intervention as needed
  • Complete all required service plans in accordance with program standards
  • Plan and facilitate monthly tenant meetings, tenant activities, and staff meetings
  • Educate participants on housekeeping, home safety, budgeting, and landlord / tenant relations
  • Maintain accurate documentation of all tenant interactions
  • Conduct home visits to assess compliance with program rules and identify needs for supportive service referrals
  • Report any suspected child neglect or abuse immediately to the Program Director.
  • Conduct and document annual and monthly apartment inspections.
  • Perform other duties, as needed
  • Qualifications :

  • 3+ years of experience in a Behavioral and / or Mental Health setting
  • High School Diploma / GED
  • Strong judgment and decision-making skills with demonstrated business acumen
  • Ability to handle sensitive issues with diplomacy and discretion
  • Excellent organizational and multitasking abilities
  • Strong interpersonal and communication skills (verbal and written)
  • Ability to maintain professionalism under pressure and operate with urgency and attention to detail
  • Desired Skills :

  • Associate's and / or Bachelor's Degree
  • Bilingual (English and Spanish)
