As a Registered Nurse Home Health for Bergen County at VNA Health Group, you’ll be an integral part of a team dedicated to providing high-quality, compassionate care to patients who need it most. You’ll make a direct impact on their lives by assessing their health needs, developing personalized care plans, and empowering patients and families with the knowledge they need to live healthier lives. This is more than a job – this is a chance to make a real difference.

Responsibilities

What You’ll Do :

• Get the Best Training :  Start with a two-week clinical orientation to learn everything you need to succeed – from key procedures to our EMR system, safety protocols, and compassionate care.
• Be the Expert :  Assess patients with complex health needs, develop personalized care plans, and ensure the best outcomes.
• Make a Connection :  Educate and guide patients and families through their healthcare journey – helping them understand their conditions and the path to better health.
• Collaborate for Success :  Work closely with a team of VNA clinicians and external providers to ensure seamless care and optimal results for every patient.
• Document with Purpose :  Record and track changes in patient conditions, updating care plans to reflect their progress and needs.

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $60,000 to USD $110,000. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education / training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
