• Evaluate and treat patients using adaptive equipment and assist with activities of daily living
• Develop and implement individualized plans of care including discharge and pre-discharge planning
• Provide patient and family education on safety, injury prevention, and transfers / lifting
• Accurately document assessments, evaluations, plans, and reassessments in Cerner
• Communicate effectively with patients, families, and interdisciplinary teams
