Position Summary :

Responsible for coordinating specialized services designed to address the specific needs for individuals who are struggling with drug or alcohol addiction. Assess clients' substance abuse history, mental health, and social support systems to develop a personalized treatment plan. Assist in the completion of documentation and the review of case records.

ESSENTIAL RESPONSIBILITIES :

Intake assessment of all participants with a substance to assess areas of need.

Assist in the development of personalized services plans that address their specific needs, goals, and challenges.

Provide information and education on the effects of substance abuse, the process of recovery, and the skills necessary for maintaining sobriety.

Provide networking and linkages between the residence, mental health providers, and alcohol and drug treatment communities. This requires both telephone and face to face contact.

Lead or co-lead psycho-educational groups, substance abuse groups and graduation groups.

Facilitate and monitor participation in various self-help groups including support groups, NA, AA.

Assist in collaborating with other healthcare professionals and community resources to provide additional support and services to clients, such as medical treatment, housing assistance, or vocational training.

Participate in weekly supervision with Sr. Program Coordinator.

Ability to count / add / subtract / manage money.

Communicate information with all staff and supervisor(s) verbally and in writing for the purpose of giving instructions, providing feedback and adjusting work practices.

Must be able to physically intervene with participants when an individual may cause physical injury to self or others, assist participant in responding to potentially dangerous situations, able to administer emergency first aid to participant.

Must be able to transport self to various sites.

Member of the treatment team responsible for assisting in the planning, developing and implementation of goals and training to further develop participants skills including but not limited to :

Budgeting

Grooming

Nutritional planning

Use of public transportation

Use of community resources

Shopping

Housekeeping

Physical health & development

Adaptive behavior / independent living

Nutrition

Vocational development

Social development

Monitor participant's progress. Tasks essential to this duties include, but are not limited to seeing, hearing and observing clients, performing manual tasks, reading and writing (documentation), lifting, climbing and descending stairs, traveling, boarding and de-boarding agency vehicles and public transportation (i.e. bus, subway) Provide training in ADL and other areas. May be required to perform these for individuals requiring.Use of telephone to make and receive calls.

Responsible to participate in evacuation and ensure resident safety in actual emergencies (i.e. fire drills). Essential functions of this duty include the ability to climb and descend fire escapes and assist others to do same. This may include both verbal and physical prompting.

Responsible for crisis intervention and incident reporting procedures.

Responsible for monitoring and recording residents' progress, identified issues, daily checklists, behavioral contracts, quarterly summaries, collateral contacts.

Meet routinely with residents and management team to review goal progress, identify participant needs and update program plan and / or utilization of review plans.

Represent the program and the agency at meetings, trainings and agency functions as directed by supervisor.

Attend all scheduled House and department meetings and mandatory in-service training.

Communicate relevant information to other staff and supervisor(s), verbally and in writing, i.e., daily communication log, telephone log, incident reports, medication incidents and behavior incidents.

Act as a role model for participants.

Maintain participant records and confidentiality of such records at all times.

Participate actively as a team member by observing, reporting and / or monitoring any physical plant related issues.

Able to lift / carry packages and boxes of supplies, as well as clients if necessary.

OTHER RESPONSIBILITIES :

Ability to work flexible schedule as work schedules and location are subject to change.

Drive facility vehicle when the need arises.

This does not limit the assignment of duties or exclude the performance of other duties not mentioned, as assigned by supervisor.

Follow-up on any aftercare / recommendations for treatment.Escort / accompany participant as needed on appointments (medical, psychiatric, substance abuse).

Assist participants in researching and obtaining community resources appropriate and applicable to their needs and goals.

Work schedules and locations are subject to change.

Qualifications

Bachelor's Degree in Social Service or a related field. CASAC and Forensic experience is preferred. Two year's experience working with Mental Health consumers. Driver's license and access to a vehicle are required.

Strong communication and listening skills to build trust with clients.

Knowledge of addiction treatment models and behavioral health interventions.

Crisis intervention skills to handle emergencies.

Compassion and empathy for individuals struggling with addiction.

Ability to develop treatment plans and coordinate care with other professionals.
