McKinley Park Care Center is seeking a compassionate and organized Social Services Assistant to support our Social Services team in providing exceptional care to our residents.

In this role, you will help ensure that residents' emotional, social, and psychosocial needs are met while supporting care planning, discharge coordination, and family communication.

Responsibilities

• Assist the Social Services Director with planning, organizing, and implementing social service programs
• Support resident assessments and care planning related to psychosocial needs
• Document and maintain social service progress notes and required reports
• Provide information to residents and families regarding Medicare, Medicaid, and financial resources
• Assist with discharge planning, including coordinating services, equipment, and community referrals
• Help coordinate ancillary services such as optometry, podiatry, dentistry, and psychiatric services
• Provide emotional support to residents and families coping with illness, disability, or transition to skilled nursing care
• Collaborate with nursing, therapy, and interdisciplinary teams to support resident well-being
• Maintain strong relationships with community health and social service agencies
• Participate in staff meetings, in-services, and training programs
• Maintain confidentiality of resident information and protected health information (PHI)

Qualifications

• High school diploma or equivalent required
• Associate or Bachelor's degree in Social Work or related field preferred
• Healthcare or skilled nursing facility experience preferred
• Strong communication, organization, and documentation skills
• Ability to work collaboratively with residents, families, and interdisciplinary teams

Physical Requirements

• Ability to sit, stand, and walk throughout the day
• Occasional lifting or moving up to 25 pounds
• Frequent use of computers and standard office equipment
• May assist with resident evacuation procedures during emergencies
