Bachelor of Science in Nursing (BSN) degree, active registered nurse (RN) license and minimum 1+ years RN experience required. Applicants who do not meet these qualifications will not be considered.

This role supports patient well-being through comprehensive assessments, individualized care planning, and hands-on clinical interventions, ensuring continuity and quality of care. Key Responsibilities :

• Perform thorough OASIS assessments and develop patient-specific care plans.
• Deliver skilled nursing care including wound management, medication administration, IV therapy, and chronic disease monitoring.
• Monitor patient progress, detect changes, and effectively communicate with physicians and care teams.
• Educate patients and families on health conditions, treatments, and preventive measures.
• Maintain accurate documentation using Home Care Home Base (HCHB) electronic system.
• Collaborate closely with therapists, social workers, and aides to coordinate comprehensive care.
• Participate in case conferences, staff meetings, and continuing education.
• Comply with all regulatory and agency guidelines to ensure quality and safety.
