LPN Remote Chronic Care Manager

Chronic Care Manager (LPN) – Fully Remote Sweeten Health (via Phamily) Job Overview : We are seeking a compassionate and patient-centered Licensed Practical Nurse (LPN) to join our care team as a Chronic Care Manager. This fully remote position plays a key role in supporting patients with chronic conditions by providing proactive, ongoing care coordination and education.
