HEALTH AND WELLNESS CASE MANAGER

MISSION : "Chileda's mission is to improve the quality of life for individuals with cognitive and behavior challenges. Chileda's goal is to help students increase their appropriate social behaviors and develop independent living skills, through an enriched learning environment and individualized behavior programming. Chileda's population includes children and adults with special needs including Autism, Traumatic Brain Injury, Attachment Challenges, related developmental delays, and intensive behavioral needs"

SUMMARY : Health and Wellness Case Managers are essential contributors to the care, coordination, and advocacy of and day students. This role involves close collaboration with interdisciplinary teams, internal and external stakeholders, and family / guardians to ensure students' healthcare needs are efficiently and effectively met. Responsibilities include maintaining accurate medical records, coordinating and attending medical appointments, training new staff, and facilitating communication with parents and staff. Successful candidates will demonstrate attention to detail, strong organizational and communication skills, and a commitment to promoting student health and well-being. This position offers the opportunity to work in a supportive and collaborative environment, contributing to meaningful outcomes for students and their families.

REQUIRED EDUCATION & EXPERIENCE :

• Associate degree in nursing or a bachelor's degree in social or behavioral sciences, such as nursing, social work, psychology, or sociology.
• : Experience working with individuals with developmental disabilities.
• : Case management experience or relevant training.
• Ability to obtain First Aid and CPR certification.
• Ability to complete Community-Based Residential Facility (CBRF) Medication Administration certification
• Strong verbal and written communication skills, as well as interpersonal skills and a passion for providing inclusive care.
• Proficiency in general office skills, including Microsoft Outlook and Word, as well as a demonstrated ability to use or learn Electronic Medical Record (EMR) systems.
• Knowledge and application of Trauma-Informed Care principles.

GENERAL HOURS OF DUTY :

Full-time, primarily 1st shift hours, Monday through Friday with flexibility to meet the needs of the students served at Chileda.
