Responsibilities :
Coordinate and manage comprehensive care plans for hospice patients, ensuring their comfort and dignity at every stage.Collaborate closely with interdisciplinary teams to deliver holistic care tailored to each patient's unique needs.Conduct regular assessments to monitor patient conditions, adjusting care plans as necessary to optimize outcomes.Provide compassionate support and education to patients and families, empowering them with knowledge and resources.Document patient care activities accurately and promptly, maintaining clear communication with all team members.Advocate for patients' needs and preferences, ensuring their voices are heard and respected in all care decisions.Participate actively in team meetings and continuous learning opportunities to enhance your skills and contribute to our shared mission.Qualifications :
Current RN license in New Jersey and Pennsylvania, ensuring you're ready to serve our community.Experience in hospice or palliative care, so you can provide compassionate, patient-centered care.Ability to conduct thorough in-home assessments, ensuring personalized care plans for each patient.Proven track record of collaborating effectively with multidisciplinary teams, enhancing patient outcomes.Strong communication skills to educate and support families, guiding them through the hospice journey.Proficiency in maintaining accurate, timely documentation, ensuring compliance and continuity of care.Ability to advocate for patients' wishes and cultural values, respecting their dignity and comfort.About Company
