JOB DESCRIPTION
POSITION TITLE : Case Manager- Level 1
DEPARTMENT : All Departments (non-clinical)
FLSA STATUS : Non-Exempt
REPORTS TO : Senior Case Manager – Level 2
GENERAL DESCRIPTION OF DUTIES
The purpose of this position is to support the Agency’s needs by providing support based on CRT’s Steps to Success model, the Case Manager is responsible for the coordination of supervision, intake, referral and discharge of customers. Daily monitoring of customer contacts and activities is a primary responsibility. The Case Manager conducts assessments of customer needs and coordinates referrals to Community Renewal Team programs and initiatives, as well as to other community resources.

ADDITIONAL JOB FUNCTIONS
MINIMUM TRAINING EXPERIENCE and health certifications
Education : associate or bachelor’s degree in the Human Services field from an accredited college or university preferred. Education may be substituted with experience on a year-by-year basis with responsible employment experience.
Minimum Years of Experience : three to five (3 to 5) years in Criminal Justice, Social Work, or related field preferred
Active Certification : Specialized training with HIV / AIDS, Drug and Alcohol Abuse and / or counseling, Domestic Violence and / or any other applicable training related to field of work. Intake and eligibility certification for all CRT programs including VITA, Energy Assistance, Early Care and Education, Eviction and Foreclosure Prevention and other CRT programs. Complete yearly mandatory training including confidentiality and HIPAA compliance. Conducts home visits. Content knowledge in employment education, financial literacy, prevention / wellness education and housing.
Knowledge of : Human Services programs; Outside Community-based resources for basic needs, Criminal Justice and the Court systems is preferred. Knowledge of substance abuse and target population helpful. Counseling background preferred. Ability to interact with people of diverse backgrounds required.
Demonstrate skills in : Ability to work with individuals from diverse racial / ethnic and economic backgrounds; Ability to maintain accurate and timely records; Strong Organizational Skills including the ability to handle multiple projects, write concise reports, plans, and correspondence; Solid Listening, verbal / written Communication, and Customer Service Skills; Ability to communicate effectively. Ability to compile written reports and maintain case notes; Proficiency in computer technology, software databases, Microsoft Office Suites are all required.
Bilingual : English / Spanish verbal & written - preferred.

Physical Ability : Tasks involve sedentary to light work, involving some reaching, handling, fingering and / or feeling of objects and materials.
Sensory Requirements : Some tasks require visual perception and discrimination. Some tasks require oral communications ability.
Environmental Factors : Tasks are regularly performed without exposure to adverse environmental conditions, such as dirt, dust, pollen, odors, wetness, humidity, rain, fumes, temperature and noise extremes, machinery, vibrations, electric currents, traffic hazards, animals / wildlife, toxic / poisonous agents, violence, disease, or pathogenic substances.
