Hospice Homecare CNA - Day Shift (Middleton / Verona Areas)

Location : Middleton and Verona Areas

Full-time 0.7 FTE (28 hours / week) Day Shift Monday-Friday 8 : 00am-4 : 30pm

Includes Weekend (every 12th) and Holiday (3 / year) rotations

Starting Pay : $18.50 + (based on experience)

Sign on bonus : $3,000

How You'll Make a Difference (Essential Functions)

The CNA Homecare (HC) is responsible for patient and family satisfaction by assisting with activities of daily living, providing light homemaker help, following the plan of care, communicating changes in condition, and documenting care. The CNA in home care (IHC) provides patient and family centered care consistent with Agrace philosophy, goals, standards, regulations and policies.

Key Responsibilities

• Assist Patients with Activities of Daily Living

Assist with ambulation, changing position, and moving to / from sitting and standing.

Perform light housekeeping such as changing linens, tidying room, doing laundry, and removing trash.

Read, understand, and follow the plan of care. Document cares and changes in patient condition in electronic health record.

Participate in meetings to plan and evaluate patient / family services.
