Hospice RN Case Manager Compensation
- The pay for this position is $56.14 per hour
- Benefits are available to full-time employees after 90 days of employment
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
Hospice RN Case Manager Highlights
The required availability for this position is Monday–Friday, 8 : 00 AM – 5 : 00 PMSome on-call shifts required : evenings, overnights, 24-hour shifts, or full weekendsTypically 5 patient visits per dayHospice RN Case Manager Responsibilities
Perform comprehensive assessments of high-risk membersCollaborate with primary care providers to develop and implement individualized care plansContinuously reassess care levels to ensure cost-efficiency and appropriatenessCoordinate non-medical support such as housing and transportation for treatment complianceManage care transitions and collaborate with physicians, social workers, discharge planners, and claims staffEngage specialty resources as needed to achieve optimal outcomesMaintain detailed documentation of clinical, functional, and fiscal outcomesPromote health education and illness preventionServe as a patient advocate while ensuring privacy and confidentialityHospice RN Case Manager Requirements
Associate's or Bachelor's Degree in Nursing or related fieldCurrent unrestricted state Registered Nurse licenseAbility to analyze complex medical information and make rational decisionsCase Management CertificationHospice RN Case Manager Preferences
