Job descriptionThe required availability for this position is Monday–Friday, 8 : 00 AM – 5 : 00 PM Some on-call shifts required : evenings, overnights, 24-hour shifts, or full weekends Typically 5 patient visits per day Perform comprehensive assessments of high-risk members Collaborate with primary care providers to develop and implement individualized care plans Continuously reassess care levels to ensure cost-efficiency and appropriateness Coordinate non-medical support such as housing and transportation for treatment compliance Manage care transitions and collaborate with physicians, social workers, discharge planners, and claims staff Engage specialty resources as needed to achieve optimal outcomes Maintain detailed documentation of clinical, functional, and fiscal outcomes Promote health education and illness prevention Serve as a patient advocate while ensuring privacy and confidentiality Associate's or Bachelor's Degree in Nursing or related field Current unrestricted state Registered Nurse license Ability to analyze complex medical information and make rational decisions Case Management Certification Experience with Home Care Home Base (HCHB) is highly preferred Must be tech-savvy if no HCHB experience

Hospice RN Case Manager Compensation
- The pay for this position is $56.14 per hour
- Benefits are available to full-time employees after 90 days of employment
- A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
