As a Home Care Nurse at Affirme, you'll provide direct care to patients in their homes — delivering the same high-quality nursing you'd expect in a clinical setting, but in a more personal environment.

Responsibilities

• Providing individualized nursing care based on each patient’s care plan
• Administering medications, monitoring vitals, and managing complex needs as appropriate (e.g., wound care, catheters, colostomies)
• Maintaining accurate and timely documentation
• Offering emotional support to clients and families
• Collaborating with the Director of Nursing to ensure high standards of care
