Requirements  24-hour deadline for all paperwork and credentials for all Ballad HM locum clinicians. They must be fully licensed with the Medical Board and DEA for the state.

MINIMUM # of SHIFTS ACCEPTABLE 7

CERTIFICATION REQUIREMENTS - Board Certification required. ABPS is not accepted

Will the facility accept alternative boards? ABPS? ABPS is not accepted

RESIDENCY TRAINED REQUIREMENTS : Residency certificate / Diplomas from an accredited University

ATLS, ACLS, PALS, BALS Required? ATLS, ACLS (Does not need to be AHA) required Preeclampsia Certificate.

STAFFING MODEL MDDO 4 day rounder, 1 Night Rounder

STAFFING MODEL NPPA 1 Day rounder

PHYSICIAN SHIFT TIMES 7a-7p, 7a-7p

CALL REQUIREMENT None

EMR SYSTEM : EPIC

ACTIVE STATE LICENSE REQUIRED TO PROCEED? Yes, DEA needs to be pending, active DL / passport, Permanent Resident card

FLU OR OTHER IMMUNIZATIONS REQUIRED? WAIVER CONSIDERED? Flu within 12 months waiver is considered. MMR and or

Titer TB within the past 12 months
