Nurse Practitioner- Johnstown, NY

Job Description

We are seeking a compassionate Nurse Practitioner (NP) to join our Skilled Nursing Facility in Johnstown, NY in a full-time role with part-time or per diem options . The NP will collaborate with healthcare teams to provide high-quality care, including assessments, treatment planning, managing medical conditions, and coordinating interdisciplinary care.

• Conduct patient assessments and develop treatment plans
• Manage acute and chronic conditions in partnership with physicians
• Order, interpret tests, prescribe medications, and document in EHR
• Participate in team meetings, family conferences, and quality improvement activities
• Ensure compliance with regulations and support nursing staff

Qualifications : NY NP license, AGNP / FNP certification, DEA registration, BLS (ACLS preferred), 1-2 years geriatrics or long-term care experience ideal.
