Job Position Description :
Are you a highly organized leader with a passion for equitable access to services and system coordination? As the Homeless Access Systems Coordinator, you will be the day-to-day operational lead for the county's Coordinated and Centralized Assessment System, known as Community Connect.
This vital role ensures the seamless and consistent delivery of services, directly supervising and training the team of Community Resource Advocates who conduct comprehensive housing assessments and make direct referrals to Washington County housing programs. You will be instrumental in developing and implementing new policies, ensuring strict adherence to protocols, and promoting strong partnerships with our community network.
Abbreviated Duties List :
- Coordinate daily operations, including efficient screening and assessment scheduling across county locations to ensure equitable access. Provide direct supervision, training, and support to Community Resource Advocates, including performance feedback and team development.
- Ensure all screenings, assessments, and referrals adhere to Community Connect policies and procedures. Monitor daily data entry activities and ensure data quality and adherence to established protocols in the statewide client databases.
- Work with program leadership to develop, implement, and communicate new policies and procedures in response to changing funding or requirements.
- Provide training and technical assistance on Community Connect assessments to staff and external community outreach partners. Maintain positive relationships and coordination with Coordinated Entry Systems partners, the Housing and Supportive Service Network, and the Washington County Shelter Systems.
- Ensure staff provide high-quality, client-centered, trauma-informed, and inclusive service interactions. Respond appropriately to client issues and concerns as needed.
Abbreviated Requirements :
A Bachelor's Degree in Social Services or a related field, PLUS at least two (2) years of experience delivering direct services to a diverse population, including client interviewing, needs assessment, and advocacy. An equivalent combination of education and experience may be considered.Proven ability to assign, coordinate, and supervise the work of others and build an effective team.Experience working with complex regulations and guidelines, including implementation, documentation, and quality control monitoring.Proven ability to work in an environment requiring frequent problem-solving and decision-making.Intermediate level computer proficiency in word processing, databases, spreadsheets, PowerPoint, and calendaring programs.Proven ability to work collaboratively with a wide range of community members, service providers, and client advocates.What Will Make You Stand Out :
