Job Description
Physical therapists (PT) provide services that help restore function, improve mobility, relieve pain, and prevent / minimize permanent physical disabilities of patients suffering from injuries or disease. The successful PT teaches and trains patients and caregivers to be an active participate in their recovery to restore, maintain, and / or promote overall fitness and health. Their patients include accident victims and individuals with other disabling conditions such as low back pain, arthritis, heart disease, fractures, hip and knee replacements, head injuries, strokes, and other neurodegenerative disease and gait disorders. Reports directly to Director of Clinical & Professional Services.
QUALIFICATIONS
- Must have graduated from an educational program for Physical Therapists
- Currently licensed as a Physical Therapist by the Illinois Department of Financial and Professional Regulation
- One year of experience in SNF, rehab or acute care OR home health physical therapy
- Demonstrate flexible and efficient time management techniques
- Ability to prioritize workload to meet productivity standards and applicable deadlines

Providing initial and ongoing assessment to determine level of functioning, including OASIS Assessment at appropriate time points.A physical therapist gathers background data, reviews referrals, selects and modifies evaluation tools, and evaluates patients determining the level of patient's physical and psychosocial functioning.Developing and revising the plan of care in consultation with the physician and other care-team members.Responsible for planning, evaluation, and treatment implementation of assigned caseload.Preparing clinical progress notes.Supervising therapy assistants and home health aides as appropriate.Evaluating outcomes.Educate patients and / or families about proper safe use and recommend methods for installation of adaptive equipment.Advocates for our patient's needsParticipates in case conferences, re-hospitalization and other committees as necessaryIdentifies opportunities for work process improvementsMaintains personal and company compliance with all state, federal, and regulatory requirements for home healthEstablishes, encourages positive working relationships with internal / external contactsPreserves, expands working knowledge of external resources to complete work processPerforms other functions that may be assignedQualifications
