South Carolina Licensed RN Case Manager

A company is looking for an RN Concierge Care Manager - Case Manager.

Key Responsibilities

Direct day-to-day high-touch member management, including treatment plan coordination for high-risk populations

Coordinate clinical services with external providers, vendors, and community services

Participate in team training and prepare for external audits and quality assurance efforts

Required Qualifications, Training, and Education

Associate's degree in nursing or graduate of an accredited school of nursing, or a master's degree in social work, psychology, or counseling

5 years of case management experience as an RN or 2 years in a health insurance environment

Active, unrestricted RN licensure or equivalent in the state of hire

Nationally recognized Case Management certification to be obtained within 2 years of hire

Knowledge of quality improvement processes and contract language
