Per Diem Clinical Review Manager (CRM) - Dual Certification Required

$40 / hour | Remote

Activa Home Health (an AccordCare Company), a licensed Medicare-certified home health agency, is seeking an experienced Per Diem Clinical Review Manager (CRM) to support our patients and clinicians. Requirements :

Dual certification in OASIS and diagnosis coding (required)

Proven experience in clinical review and home health documentation

Ability to assist the team with overflow charts , especially during month-end and PTO coverage

Strong skills in reviewing and completing 30-day summaries

Experience with face-to-face review with coding and QA teams

Knowledge of creating 485 forms for SOCs and updating recertification OASIS forms

Ability to review post-facility discharges and create post-hospital orders

Experience in reviewing and updating coding for OASIS follow-ups (SCIC)

Strong communication and collaboration skills with field clinicians regarding OASIS documentation updates

Ability to provide education and follow-up as needed to ensure accurate and compliant documentation

Responsibilities :

Support team with overflow charts and coverage needs

Conduct face-to-face reviews with coding and QA teams

Create and update 485 forms for SOCs and recertifications

Review post-facility discharges and create post-hospital orders

Update coding for OASIS follow-ups (SCIC)

Collaborate with field clinicians on documentation, provide education, and ensure follow-up
