About The Position

Job Title : Physical Therapy Assistant

Profession : Rehabilitation Therapy

Duration : 13 weeks

Shift : Monday to Friday

Hours per Shift : 32 hours guaranteed

Experience : New Graduates okay; SNF experience preferred

License : Active physical therapy assistant license required

Certifications : Covid test required 72 hours prior to start date with results issued 48 hours prior to start date

Must-Have :

• Completed prescreen form
• Resume
• License verification
• Three references
• County criminal background check (7 years for all counties lived and worked)
• Federal criminal background check (7 years or more)
• Multi-state criminal search (7 years or more)
• Nationwide sex offender search (7 years or more)
• HHS / OIG search
• GSA search
• Social Security number trace
• Two-step PPD or Quantiferon or proof of positive PPD and chest X-ray
• Oregon state background check

Description :

This position requires working two days at one facility and three days at another site.

Candidates must ensure the prescreen form is completed and attached.

The resume and license verification also need to be attached.
