Job descriptionProvide one-to-one patient care in a home environment. Deliver specialized nursing care to medically complex adult patients. Perform patient assessments and coordinate care. Promote health, teach, and train patients and / or family members. Work one weekend per month. Qualifications of the Nurse :

- Must have a good standing license (LPN) in the state where the clinician will practice.
- Current CPR certification.
- Willing to travel and visit patients in their homes.
