Position Overview
The Community Health Worker provides community-based care coordination and support services by building relationships and linking residents to local programs, services, and opportunities. This role will focus on identifying, engaging, and connecting community members to available health and social services and resources to address needs such as housing, food, education, employment, transportation, and more are Management. While this role may serve all community members, there is a focus on supporting Medi-Cal members. The ideal candidate will have lived experience, familiarity with Fresno and / or Madera communities, and ability to relate to people from all backgrounds.
