Description :

The Home Care Coordinator is responsible for hospital discharge planning.

This role involves verifying that the patient has the necessary devices needed for nursing and home care, such as walkers and commodes.

The RN will set up home care services if needed and advise on appropriate support organizations in the community.

Patient and family education is also a critical part of this role.

The position acts as a discharge planning resource to hospital staff involved in home care planning.

The Home Care Coordinator facilitates continuity of care for patients transitioning to home-based services.

This role serves as an on-site expert in home-based care.
