Job descriptionResidency : Successful completion of an ACGME or AOA-accredited residency program in Emergency Medicine, Family Medicine, or Internal Medicine. If IM or FM, they MUST have 2 current years AND currently working in an Emergency Department. Board Certification : Board Certified or Board Eligible (with the achievement of certification within 5 years) leading to certification in their specialty by the American Board of Medicine (ABEM), Family Medicine (ABMS) or American Osteopathic Board of Medicine (AOBM). Life Certifications : Must have all current certifications listed on the presentation, including NIH Stroke Scale (NIHSS), ACLS, and PALS required for all Emergency Medicine providers. Immunizations : PPD / FLU / COVID / TB (TB is for initial appointment only) CME : 24 months CASE LOGS : 24 months CV : Must have ALL hospital affiliations and work experience listed on the CV in Month / Year format for Start and End Dates, including city and state name. Board Certified Board Eligible DEA NPI Missouri Attached CV in MM / YY to MM / YY format with Provider Phone Number, Email, and Home Address. Any gaps of over 30 days must be explained on CV. Pre-Application (EPA) NPDB submitted with CV if provider answered YES to any of the Negative questions. Board Certification(s), Certifications, Procedures (For IPS providers). Available to Start, Shift Availability, and willingness to work day / night / weekends. License status of state presenting? Active or In Process (Must provide proof application) or Apply upon Approval. Other Active State License(s). DEA(s) & their status. Malpractice / Disciplinary : Must provide a detailed explanation with the presentation. Call Interview Availability for the next 10 business days. Phone number availability. Airfare, Lodging, and Rental details. Affirmation of willingness to obtain a state-specific DEA for this assignment. Affirmation not to change the DEA address during the duration of the candidate's privileged time with the facility through EVPS.
