Unit : LPN - LTC

Assignment & Scheduling Details :

Start Date : ASAP Start (Can accommodate a midweek start)

Duration : 13 Weeks

Shift : Evenings , 8- hour shifts ( 02 : 00 PM-10 : 30 PM ) or Nights , 8-hour shifts (10 : 00 PM - 06 : 30 AM)

Schedule : Strictly 5 shifts per week (5 / 8s) .

Holiday Requirement : Must be available to work either Thanksgiving or Christmas

Guaranteed Hours : 40

Financial & Compensation Structure :

Local Traveler Rate : Local candidates residing within 50 miles are accepted, but will be subject to a rate decrease

Premium Pay :

On-Call Pay : $8.00 / hour

Callback Pay : $5.00 additional (paid on top of regular rate)

Charge Pay : $3.00 additional (paid on top of regular rate)

Overtime (OT) Pay : $10.00 additional (paid on top of regular rate)

Job Summary :

A Long Term Care facility in Amsterdam, NY, is seeking an experienced Licensed Practical Nurse (LPN) for a critical 13-week night assignment. This role is strictly scheduled as 5x8s and requires a professional with recent LTC experience and a strong background as a traveler. The ideal candidate must be immediately available for an ASAP start and able to navigate the facility's charting system, Matrixcare.

Required Qualifications & Certifications

Experience :

Minimum 1 year of recent experience in a Long Term Care (LTC) setting is required.

Travel experience is required , preferably showing evidence of contract renewals.

Licensure : Current and active New York State (NYS) LPN License is required.

Certifications : Current Basic Life Support (BLS) certification is mandatory.

EMR / Charting : Proficiency with the Matrixcare charting system is required.

Exclusions : The facility will not consider previous staff members as travelers.
