Location : This position involves conducting patient home visits in the Downtown Orlando, Pine Hills, West Orlando, Ocoee, Winter Garden, and Windermere areas, including the following zip codes : 34760, 34787, 34734, 34786, 34761, 32818, 32835, 32819, 32811, 32808, 32810, 32714, 32804, 32805, 32839, 32806, 32801.

As a Home Health Physical Therapist Care Manager, you will play a crucial role in coordinating and directing patient services according to their specific needs. Your responsibilities will include :

  • Managing a caseload of home health patients requiring physical therapy.
  • Performing comprehensive assessments and developing individualized care plans in collaboration with patients, families, and physicians.
  • Conducting therapy sessions and monitoring patient progress to achieve set goals.
  • Educating patients and their families to foster independence and safety at home.
  • Maintaining accurate documentation and communicating effectively with healthcare teams.

    • Your qualifications should include :

  • Master's Degree in Physical Therapy
  • A minimum of 1 year of relevant clinical experience in physical therapy.

    • Preferred qualifications :

  • Doctorate Degree in Physical Therapy
  • Experience in a Medicare-certified home health agency as a care manager.
  • Certification as an Oasis Specialist-Clinical (COSC)
  • Home Health Care Manager Certification.
