Compact Licensed Disease Management Nurse

A company is looking for a Disease Management Nurse - Remote.
Key Responsibilities
Provide telephonic and omni-channel support to participants in the Disease Management program
Educate participants on their chronic conditions and assist in developing personalized care plans
Identify participants at risk for increasing acuity and facilitate communication with their Primary Care Providers as needed
Required Qualifications
Current Registered Nurse multi-state compact license; BSN preferred
Minimum of 3 years recent clinical experience
Basic knowledge of physiology and chronic disease management
Ability to communicate effectively with members and healthcare professionals
Basic computer skills and ability to type at least 40 WPM
