Home Health RN Registered Nurse (PRN) – Empath Home Health

Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Home Health RN to provide skilled nursing care for patients in their homes throughout east / south Hillsborough County (Brandon, Riverview, Valrico, Seffner, Bloomingdale, Apollo Beach, Lithia, and surrounding areas). In this PRN role, you’ll deliver individualized care, build trusted patient relationships, and help people manage their health with dignity and independence.

This is an exciting opportunity to be part of a market expansion , serving patients in their homes with the flexibility to set your own schedule and grow with us as we continue to build our presence in the community.

Locations : Home health visits in west Pasco County (New Port Richey, Hudson, Holiday, Trinity, Odessa, and surrounding areas). If desired, visits may also be available in east Pasco, Pinellas, and / or Hillsborough counties!

Schedule : PRN role offering 5-10+ visits per week. Required weekend / holiday on-call rotations. Potential for full-time in the future if desired!

What You’ll Do

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled nursing services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care
