Job Description & Requirements Specialty : Home Health Discipline : RN Start Date : 12 / 22 / 2025 Duration : 13 weeks 40 hours per week Shift : 8 hours, days Employment Type : Travel Requirements : Shift : 8-hour Days (No 4x10s or 3x12s) Patient Ratio : 1 : 1; up to 6 visits / day (30 productivity points) Experience Required : Minimum 1 year Skilled Home Health Certifications : BCLS Locals : Accepted (within 50–75 miles, same rate) Pending License : Accepted Guaranteed Hours : Facility may call off 1 shift per pay period Special Requirements Must have own transportation Patient assignments grouped to minimize drive time; occasional max radius 50 miles Must have COVID primary series 1 booster OASIS experience required (must be called out if candidate does not have it) Required Skills Documentation : Start of Care, Recertification, Resumption of Care Routine Visits Admission Assessments Discharge (including safety / non-compliance) Professional Skills : Infection control Medication administration (IM, SQ, ID, IV infusions) Medication assessment & management Pain assessment (verbal / nonverbal), PCA management Wound care PJ -HC (3058286) About LanceSoft Established in 2000, LanceSoft is a Certified MBE and Woman-Owned organization.
