Job Description

Responsible for delivery of care management services to designated populations. Responsible for managing the daily activities and outcomes of the Care Management staff. Works with other leaders on ensuring adequate staffing and training to meet clinical and program needs. Responsible for measuring staff performance. Responsible for managing strategic relationships with client groups and community leaders as needed.

Job Responsibility

• Works with site management to develop new programs and systems that will enhance the area operations. Analyzes area-specific data to identify trends and opportunities to improve care and/or service delivery

• Active partner in day-to-day operations of Care Management and development of new initiatives.

• Implements and facilitates disease or utilization-specific databases and processes.

• Assures program components are up to date.

• Maintains professional competency and future development by attending continuing education conferences and seminars.

Job Qualification

• Master's Degree in Nursing or related field required, or equivalent combination of education and related experience. Bachelors or Masters must be in Nursing.

• Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.

• 6-8 years of relevant experience and 2-5 years of leadership / management experience, required.

• Patient Review Instrument certification required.

The salary range for this position is $98300.00-$170100.00/year
