Treatment Foster Care Coordinator
Pay : 43,000-45,000; Licensed (LSW) : 46,000-48,000
This is a community based position. Travel and home visits are required.
Treatment Coordinators with National Youth Advocate Program work in the community as part of an integral service team.Under the general supervision of the Clinical Supervisor or designee, this position works closely with youth, family, foster parents, and community partners, providing advocacy and support toward the ultimate goal of living safely and successfully in the community.

Responsibilities
Provides appropriate individual, group, and family mental health services, and other evidence basedclinical servicesas required.Travels daily, to provide community basedservices to, and on behalf of, youth andfamilies incompliance with organizational, contract, and regulatory requirements.Coordinates and monitorsclinical servicesfor the youth and family in the community, including mental health,specialeducational services, medication management, andpsychological services.Regularly reviews and modifies youth-basedplans.Establishes andmaintains strongrelationship with foster parents andfamilies of origin.Assists in family reunification and stabilizationefforts with the youth's primary family or other permanent living arrangement consistent with the youth's case plan.Provides safetyassessment,crisis response, and behavioral stabilization servicesas required.Minimum Qualifications
A Bachelor's Degree in Social Work from an accredited institution.LSW or license-eligible, preferred.2 years of work experience working in direct service with youth and families preferred.A willingness to work flexible and non-traditional hours as needed.Proficient use of desktop and laptop computers, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs.Driving and Vehicle Requirements
