Shift : Mon, Tue, Thur, Fri 7 : 30a-5 : 30p,

Start Date : As soon as credentialed

End Date : Ongoing

Logistics covered : Mileage, lodging, flights if needed

Details :

We do NOT have a Pediatric population- High Geriatric Population.

Outpatient Internal / Family Practice Physicians.

Shift : Mon, Tue, Thur, Fri 7 : 30a-5 : 30p, No weekend / On-call

Setting : Outpatient Clinic

Duties : Family Practice

Days per month : 20

Required Procedures : toenail removals, lesion removals, Pap Smears, Joint injections. (negotiable)

EMR : Cerner

Support Staff : Medical Assistant / Triage RN / Front Office Staff / Clinic Manager

Submission Requirements

Board Certified - REQUIRED

Active AZ License - REQUIRED

Fellowship - REQUIRED

BLS, DEA - REQUIRED

Availability at time of name clear - REQUIRED

Malpractice - Clean Preferred
