Job Summary
Responsible for assisting the care team (provider, medical assistant, behavioral health provider, etc.) by coordinating services for patients who are part of the assigned panel, especially those with serious, complex, chronic or psychosocial issues.
Job Duties
- Provide an effective communication link between patient, medical staff, behavioral health staff, rehabilitation facilities, hospitals, etc.
- Schedule, coordinate and track referrals for diagnostic testing and specialty care.
- Assist in coordination of care with other providers in the community, ensuring that information goes when and where it is needed.
- Facilitate provider communication at regular intervals throughout patient"s hospitalization or stay at other facilities.
- Maintain ongoing communication with discharge planners, case managers, and care coordinators at facilities to which patients are periodically admitted.
- Ensure all patients are tracked and data entered into systems for follow up and reporting.
- Coordinate with medical staff to ensure that case management services are provided to patients with complex medical and / or psychosocial problems.
- Handle protected health information in a manner consistent with HIPAA.
- The Referral Coordinator will have access to protected health information during the course of work activities to communicate medically necessary information to other health care providers to facilitate care for the patient.
- Applying the minimum necessary standard for HIPAA, the employee will have access to the full medical record, the practice management system, and the chronic disease registry system.
- Consistently demonstrate and uphold CCCHC's principle of providing quality health and human services to medically underserved and low-income populations in a culturally sensitive manner.
- Punctuality and Attendance : This is an essential job duty for CCCHC's employees given the impact on patients.

Bilingual in English / Spanish preferred (or English / Vietnamese if assigned to Garden Grove).Current CA LVN or RN license requiredHigh School diploma or GED required.Two years of college desirable.Prior case management experience preferred.Prior experience working with underserved populations preferred.Working Conditions
Normal clinic environment with physical requirements including : hearing, seeing, speaking, feeling, reaching and repetitive motions. Walking and / or standing is routinely required.This position may require sitting for long periods, including work at a computer for extended periods daily.Ability to exert up to 40 pounds of force to lift, carry, and move objects.Ability to handle multiple tasks while being tactful, diplomatic, and empathetic.Ability to organize and schedule work effectively.Ability to communicate effectively with patients, customers, managers and other personnel, in person and on the telephone.Ability to pay close attention to detail.Ability to maintain composure under urgency and / or time constraints.Ability to work under pressure in a fast-paced environment and maintain patient confidentiality.Ability to work at a computer terminal for extended periods daily.Central City Community Health Center offers a dynamic work environment with competitive salaries and benefits.
