Position Summary

We are seeking a self-starting, growth-oriented Clinical Pharmacist to join our team. The Clinical Pharmacist will support both dispensing and clinical care, with a primary focus on optimizing medication use and improving health outcomes through direct patient care, collaboration with the care team, and implementation of clinical pharmacy services. This role will evolve based on patient needs and the pharmacists professional interests, with opportunities to develop and expand services within chronic disease management, population health, and quality improvement initiatives.

Essential Responsibilities

In addition to traditional pharmacy functions, the Clinical Pharmacist is responsible for :

Clinical Care & Patient Management

• Leading and expanding clinical pharmacy services, including chronic disease management (e.g., diabetes, hypertension, asthma), medication therapy management (MTM), and comprehensive medication reviews (CMRs).
• Collaborating with medical providers to identify and resolve medication-related problems, optimize therapeutic outcomes, and develop patient-specific care plans.
• Participating in interdisciplinary care teams and population health programs, including quality improvement and value-based care initiatives.
• Assisting in transitions of care by reconciling medications, educating patients, and reducing hospital readmissions.
• Conducting patient education and counseling on disease states, medication adherence, side effects, and lifestyle interventions.
• Administering immunizations and prescribing contraception under collaborative agreements and state protocols.
