Riverwood Center is located in beautiful Southwest Michigan with staff at seven sites across Berrien County. Responsibilities :

Provides case management services in the community to consumers with severe mental illness

and their families

Assists in planning, linking, advocacy, coordination and monitoring; to assist consumers in

gaining access to health services, financial assistance, employment, education, social

services and natural supports

Assesses needs, develops and monitors Person Centered Plans and documents treatment in

an accurate and timely manner

May provide coordination of care between the criminal justice system and consumers

with mental illness that participate in the Berrien County Mental Health Court and / or

the prisoner re-entry initiative

Qualifications :

Bachelor Degree in an appropriate mental health discipline.

One or more years experience providing treatment to individuals mental illness.

Excellent computer and interpersonal skills, punctuality, ability to organize and manage

several projects / tasks simultaneously, and work independently.

Must have reliable means of transportation for community outings and transporting consumers.
