The hospital is looking for NP - Hospitalist Nurse Practitioner in Goodyear, AZ

JOB DESCRIPTION :

SUBMISSION REQUIREMENTS

Must have clean background and malpractice
Must have ATLS
Must have AZ license and DEA
Must have ACNP or certification
1st assist & trauma experience preferred
At least 1 year of experience in Trauma / ED and ICU Submission Requirements

AS A REMINDER :

All candidates must have met the minimum shift availability listed on the job.

CV (Curriculum Vitae) must have all dates in MM / YY to MM / YY format

Work History must be in Sequential order with most resent-on top

Any Gaps over 30 days must be explained on the CV

EPA (Envision Pre-Application) (Please use the attached EPA. It is also in Trio under your Name & then click on Help)

All Licenses, Boards and Certs with expiration dates must be listed on CV

Anyone with malpractice must provide a NPDB

Anyone with legal issues must provide supporting documents

Availability

1st date would they be available?

How many shifts would they be able to do per month?

What month is their schedule completely open?

Any shift preferences (Day, Night, Blocksetc.)?

Any dates in the next 3-6 months they cannot work?

Any Holidays in the next 3-6 months would they be open to work.?

ABOUT THE FACILITY

200-bed facility

Level I Trauma Centre (State), Level II (ACS)

ED Volume : 55k; 33 beds

14 days of EGS call per month

Inpatient census : 20 / day

20 ICU beds, additional 10 medical ICU beds

CERTIFICATION REQUIREMENTS

ATLS

BLS

Board Certified

STATE LICENSE REQUIREMENTS

Arizona
