Key Responsibilities

Support patient enrollment and eligibility verification for Chronic Care Management (CCM)

Conduct patient outreach and engagement through text and phone communication

Document patient interactions and ensure compliance with billing and HIPAA standards

Required Qualifications

Certified Medical Assistant

1-3 years of healthcare experience

Experience in chronic disease or nephrology preferred

Ability to manage workflows in EHR and CCM platforms

Intermediate skills with MS Office Suite
