Virginia Healthcare Job Details

Location : Glen Allen, VA (serving a 50-mile radius, including Williamsburg and Newport News)

Setting : Hospice / Home Health

Schedule : Monday – Friday, 8 : 00 AM – 5 : 00 PM

Shift Duration : 8-hour days

Guaranteed Hours : 40 hours per week

Assignment Length : 8 weeks (Start : 11 / 24 / 2025, End : 1 / 18 / 2026)

On-call / Weekend : No on-call or weekend requirements

Patient Coverage : 5-10 patients in Williamsburg / Newport News area until new staff onboarding is complete

Productivity : 25 points per week

EMR : HCHB preferred

Estimated Weekly Salary : $2070-$2130

Social Worker Qualifications

Master's degree in Social Work (MSW)

Minimum 1 year of home hospice experience required

Ability to travel within a 50-mile radius from Glen Allen office

Strong communication and organizational skills

Familiarity with HCHB EMR preferred

Hospice Social Worker Responsibilities

Deliver compassionate social work services to hospice patients and families

Conduct psychosocial assessments and create individualized care plans

Support patients in Williamsburg and Newport News until new staff onboarding is complete

Collaborate with interdisciplinary healthcare team members for holistic patient care

Maintain accurate documentation and meet productivity standards

Travel to patient homes within assigned Virginia territory
