About This Position : The Licensed Medical Social Worker plays a vital role in collaboration with a compassionate care team, delivering essential support services to patients and their families. This position is instrumental in ensuring that all staff members provide high-quality care throughout the patient's journey, from admission through discharge and into aftercare. Adherence to department-specific policies and procedures, as well as maintaining assigned competencies, is essential.

Main Responsibilities :

• Contribute to department goals and objectives while adhering to policies, procedures, quality, and safety standards. Ensure compliance with governmental and accreditation regulations.
• Assess the psychosocial needs of patients and their families, particularly concerning terminal illnesses.
• Work collaboratively with the interdisciplinary team to develop and implement a comprehensive care plan.
• Document psychosocial assessments, goals, interventions, and patient family responses effectively.
• Conduct assessments and referrals for ongoing bereavement services.
• Support patients' unique cultural and spiritual beliefs by facilitating connections to clergy or chaplains as needed.
• Enhance the caregiving environment by linking patients, families, and caregivers with community resources.
• Provide interventions targeting specific symptoms such as fear, grief, depression, and anger.
• Screen for psychopathology and provide appropriate education and interventions.
