About This Position : The Licensed Medical Social Worker plays a vital role in collaboration with a compassionate care team, delivering essential support services to patients and their families. This position is instrumental in ensuring that all staff members provide high-quality care throughout the patient's journey, from admission through discharge and into aftercare. Adherence to department-specific policies and procedures, as well as maintaining assigned competencies, is essential.
Main Responsibilities :
- Contribute to department goals and objectives while adhering to policies, procedures, quality, and safety standards. Ensure compliance with governmental and accreditation regulations.
- Assess the psychosocial needs of patients and their families, particularly concerning terminal illnesses.
- Work collaboratively with the interdisciplinary team to develop and implement a comprehensive care plan.
- Document psychosocial assessments, goals, interventions, and patient family responses effectively.
- Conduct assessments and referrals for ongoing bereavement services.
- Support patients' unique cultural and spiritual beliefs by facilitating connections to clergy or chaplains as needed.
- Enhance the caregiving environment by linking patients, families, and caregivers with community resources.
- Provide interventions targeting specific symptoms such as fear, grief, depression, and anger.
- Screen for psychopathology and provide appropriate education and interventions.
Qualifications :
A Master’s degree in social work is required.Preferred experience in healthcare and home health care settings.Completion of a hospice training program is preferred.Licenses and Certifications :
A valid LMSW (Licensed Medical Social Worker) certification from the state is required.This position offers a competitive hiring range of $29.15 - $48.99 per hour.
