Position Summary : \n\nThe RN Case Manager is responsible for coordinating and managing patient care across the continuum to ensure quality, cost-effective outcomes. The role involves assessing patient needs, developing individualized care plans, facilitating communication among interdisciplinary team members, and ensuring appropriate utilization of resources and services.\n\nKey Responsibilities : \n\nAssess patients’ clinical and psychosocial needs to develop, implement, and monitor individualized care plans.\n\nCoordinate and oversee all aspects of patient care from admission through discharge and post-discharge follow-up.\n\nCollaborate with physicians, therapists, social workers, and other healthcare professionals to ensure continuity of care.\n\nEducate patients and families about treatment options, resources, and self-care strategies.\n\nMonitor and evaluate patient progress and outcomes; modify care plans as needed.\n\nEnsure compliance with organizational policies, accreditation standards, and federal / state regulations.\n\nAdvocate for patients’ needs while balancing clinical and financial outcomes.\n\nDocument all case management activities accurately and timely manner in the electronic medical record (EMR).\n\nParticipate in multidisciplinary team meetings and quality improvement initiatives.\n\nIdentify barriers to care and coordinate services to overcome them (e.g., transportation, insurance authorization, referrals).\n\nLicensure / Certification : \n\nCurrent Registered Nurse (RN).\n\nCase Management certification (CCM, ACM, or similar) preferred.\n\nExperience : \n\nMinimum [2] years of clinical nursing experience (medical-surgical, critical care, or home health preferred).\n\nPrior experience in case management, utilization review, or discharge planning strongly preferred.
