Job Description :

Sunbelt Staffing is hiring an experienced Home Health RN in New Haven, CT. The nurse will provide skilled patient care in the field, maintain accurate OASIS documentation, and support patient independence at home. This role is ideal for nurses who thrive in community-based practice.

Key Responsibilities :

Perform start-of-care assessments, revisits, and discharge planning

Document all visits using OASIS in Homecare Homebase (HCHB)

Provide wound care, injections, medication management, and patient teaching

Communicate with providers and coordinate interdisciplinary support

Educate patients and caregivers on home safety and self-care

Qualifications :

Current RN license in Connecticut

At least 2 years of nursing experience, including 1 year in home health

OASIS and HCHB proficiency required

Active BLS certification

Valid drivers license, auto insurance, and reliable transportation

Able to pass background check, drug screening, and Joint Commission onboarding
