Job descriptionProvide direct nursing care to patients / residents in accordance with the plan of care. Administer medications and treatments as prescribed by physicians. Monitor patient health and report any changes in condition to supervising RN or physician. Assist with activities of daily living (ADLs) such as bathing, dressing, feeding, and mobility. Maintain accurate and timely documentation of care provided. Collaborate with healthcare team members to ensure optimal patient outcomes. Follow infection control and safety procedures at all times. Communicate effectively with patients, families, and staff. Current and valid LPN license in the state of New York . CPR / BLS certification (required). Minimum of 1 year of nursing experience. Strong clinical, communication, and organizational skills. Ability to work independently and as part of a team. Flexibility to work varying shifts as needed.
