Registered Nurse Care Coordination Lead

A company is looking for a Registered Nurse Care Coordination Team Lead (RN) - Population Health.

Key Responsibilities

Provide clinical oversight and management of the Population Health team

Lead training and orientation of new staff while monitoring case management productivity and quality

Collaborate with interdisciplinary teams and lead patient outreach and health promotion services

Required Qualifications, Training, and Education

Registered Nurse license (required)

Associate's Degree in Nursing (required); Bachelor's Degree in Nursing (preferred)

2 years of Care Management experience

Certified Care Management Certification (preferred)

Experience with Epic Healthy Planet and knowledge of population health strategies
