Overview
We are seeking a compassionate and skilled Home Health Physical Therapist to join our Phoenix team. In this role, you will deliver personalized, evidence-based therapy services that help patients regain mobility, independence, and quality of life in the comfort of their homes. As a key member of the interdisciplinary care team, you'll collaborate with nurses, social workers, and other clinicians to ensure coordinated, patient-centered care that achieves meaningful outcomes.
Key Responsibilities
Patient Care Excellence
- Provide individualized physical therapy interventions that improve mobility, strength, and overall functionality.
- Incorporate evidence-based practices to optimize patient recovery and care outcomes.
Operational Performance
Develop and adjust therapy care plans to align with patient goals and evolving needs.Monitor progress, track outcomes, and document thoroughly in the electronic health record (EHR).Manage caseload efficiently while maintaining high standards of care.Collaboration & Support
Work closely with interdisciplinary team members to ensure holistic care delivery.Mentor therapy assistants or junior clinicians, fostering professional growth.Patient Engagement & Outcomes
Build trust with patients and families by providing compassionate care and clear education.Promote long-term improvements in patient health while reducing hospital readmissions.Qualifications
Bachelors or Doctorate in Physical Therapy (DPT) from an accredited program.Active state licensure as a Physical Therapist.Minimum 2 years of physical therapy experience, preferably in a home health setting.Strong clinical knowledge of therapy practices, regulations, and compliance requirements.Excellent communication, problem-solving, and time management skills.Proficiency with EHR systems and telehealth tools.Familiarity with Medicare and Medicaid home health guidelines.Work Environment
Primarily in patients' homes within the Phoenix service area; occasional office visits for meetings or training.Fast-paced, patient-focused environment requiring adaptability and organization.Success in This Role Will Be Measured By :
